All treatments must be PAID IN FULL at the time of service
This is a dental discount plan and is not dental insurance
Membership is non-transferable nor refundable
It cannot be combined with any other offers
Rates are subject to change annually
Cosmetic procedures are excluded
For orthodontic treatment, the participant must remain a plan member the entire duration of the treatment
